Dementia Capable Wisconsin-New Partnerships in Dementia Care

Grant Objectives

  1. Provide effective supportive services to people with dementia living alone at home by developing and testing an Emergency Medical Services (EMS) Visitor Program in two Wisconsin communities. 
  2. Treat and support individuals with moderate to severe dementia by working with two home health nursing services to train caregivers to handle common medical concerns.
  3. Improve programs and services for individuals aging with intellectual and developmental disabilities (I/DD) with Alzheimer’s disease and related dementias by training I/DD specialists and healthcare professionals to identify and support persons with I/DD and dementia, their families, and caregivers. 
  4. Enhance the skill base of dementia support specialists and dementia care organizations in Wisconsin to support family caregivers in managing the behavioral and psychological symptoms of dementia.
